Each form is formatted for online completion and can be emailed directly to email@goodshepherdfund.org.org. Small Business Information Request
Are you interested in starting a small business? To see if your Trust funds can be used to help with your start-up, we need a bit more information from you. Please fill out and return this form, along with a signed copy of the ‘Self-Employment and Benefits Eligibility’ form.
Care Services Request Form
This form is used to request funds for Care Services, such as Respite, Attendant Care, or Companion Services, to cover the cost of services. If you need assistance with daily activities or temporary relief for a caregiver, simply fill out and submit this form to get the support you need.
ABLE Account Transfer Application
This form is used to request a transfer of funds from your Trust to an ABLE account. An ABLE account is a savings account that allows you to save for qualified disability expenses (QDEs) without affecting your eligibility for benefits like SSI and Medicaid. Please remember, submitting this form does not guarantee approval of the transfer.
Waiver & Consent for Grantor Beneficiary Aged 65 or Over
This form is used to provide informed consent and acknowledge the liability terms for Grantors or Beneficiaries aged 65 and over. By filling out and submitting this form, you confirm that you understand and agree to the outlined terms related to your Trust.
